INFORMATION FOR ACCESSING HOME HEALTH CARE SERVICES
Spectrum Home Health and Hospice Care is committed to working with physicians and other healthcare providers, as well as clients and family members, to make the referral process as simple and convenient as possible.
Below you will find helpful information regarding accessing our services and the benefits of choosing Spectrum for your home healthcare needs.

- Referrals to Spectrum can be made by calling (413) 567-4600, 24 hours a day, 7 days a week. Referrals can also be faxed to (413) 567-3782 during regular business hours.
- Once a referral is made, our home healthcare services typically begin within a 24-hour period.
- Our interdisciplinary team has the skills and knowledge to provide whatever home healthcare service is needed - from skilled nursing and occupational, physical and speech-language therapy to assistance with activities of daily living and counseling/ assistance with accessing community resources.
- Spectrum works closely with the client, as well as their physician and family, as appropriate. We strive to ensure that each client has a primary clinician who will work with him or her through the entire rehabilitation period, adapting the treatment program to accommodate the everyday activities of home.
- All data collected from our Telemedicine program is made available to physicians in a timely manner (daily, weekly, or as requested).
- We accept Medicare and Medicaid, as well as most other major insurances. If you have any questions regarding insurance, please feel free to call us and we would be happy to answer any of your questions.
Spectrum serves the towns of Longmeadow, East Longmeadow, Springfield, West Springfield, Agawam, Chicopee, Hampden, Ludlow, Westfield and Wilbraham.
