JGS Family Medical Care and House Calls is a primary-care medical practice that promotes healthy and independent aging. Located on the Harry and Jeanette Weinberg Campus of Jewish Geriatric Services, the practice is led by Udaya Jagadeesan, MD, a specialist in Geriatric Medicine. Gregg Wolff, MD and Nurse Practitioners Sue Cournoyer and Kristine Wailgum complete this team of professionals, dedicated to providing outstanding care with a personal touch. Family Medical provides home visits to the frail elderly and homebound patients who are unable to come to a physician office. We accept Medicare, Medicaid and most major health insurance plans.
